Guideline for Donation

Requirements And Conditions (Before Dispatching The Consignments Of Donations) For Smooth And Expeditious Customs Clearance & Acceptance Of The Consignments
The Donation Will Be More Effective If The Donor Can Follow The Steps Below.

Please try to get products registered in recognized regulatory bodies such as US FDA, EMA, MHRA, TGA Australia or registered products of NMRA(National Medicines Regulatory Authority) Sri Lanka.

If you can arrange the supply through the local agent it will be much easy.

Requirements & Conditions For All Medical Items (Pharmaceuticals, Surgicals & Diagnostics)

(1) Documents to be completed (at the earliest before the time of shipment).

i. Custom purpose original invoice and packing list including

  • Director Medical Supplies Division as consignee
  • MSD VAT registration No. ( 409031765 – 2525 )
  • Invoice No. & date
  • Contact details of the Donor
  • Notify Party - any personal engaged in locally coordinating or consignee

ii. Item description in full, including

  • images of the label
  • images of outer pack
  • catalogs etc

iii. Details of item the in full, including

  • quantity
  • expiry date
  • unit price
  • Total price etc.

iv. Certificates to be submit (if possible )

  • Certificates of analysis
  • Copy of product registration at NMRA (If product registered at NMRA)

v. Waiver of registration certificate (WOR) from NMRA if product is not registered (if possible).

vi. Certificate of origin

Requirements And Conditions For The Other Medical Items

vii. Fumigation certificate (for used medical items that should undergo quarantine processing)

viii. Catalogs, manuals or detail description (For machines used for medical purpose- Bio Medical Equipment)

ix. Certificate on the conditions of usability / usage or maintenance from local agent or recommendation from director Bio-Medical engineering. (For machines used for medical purpose)

( 2 ) Submission of documents at the time of shipping which needed to initiate the custom clearance file by MSD if consignment is clearing by MSD.

  • Documents contained in (1) above as required
  • Original Bill of Lading (B/L) or Air Way Bill (AWB) Arrival notice
  • Shippers invoice for freight charges ( For Duty + Tax )
  • For the containerized cargo, ‘Damage free Container’ certification
  • Any other special documentary requirements depending on the nature of the items ( Items with radiation source, Import restricted items and Items controlled on quality/local standards
  • Any other Conditions related to donating the consignment

Eg. Copies of meeting minutes with Senior Officers of State ministry or ministry of health of Sri Lanka.
